- Deliver high-quality assessment and interventions aligned with CDNT principles.
- Work within an interdisciplinary team to support children and families with complex needs.
- Provide therapeutic, practical and emotional support to service users and their families.
- Contribute to family-centred planning and integrated care pathways.
- Complete holistic social work assessments for individuals, families, and groups.
- Provide counselling, guidance, and practical support.
- Manage and prioritise your own caseload.
- Collaborate effectively with families, carers and interdisciplinary team members.
- Participate in the Assessment of Need (AON) and Preliminary Team Assessment (PTA) processes.
- Maintain high standards of practice in line with legislation, policies, and professional guidelines.
- Promote safeguarding, risk awareness, and safe working practices.
- Contribute to education and training for families, carers, and students.
- Registered (or eligible for registration) with the CORU Social Work Registration Board.
- Strong interpersonal and communication skills.
- Demonstrated competence in social work practice.
- Relevant ICT skills.
- Full, clean driving licence, with access to own car.
