Duties include but are not limited to effective supervision and coordination of the team of programme facilitators and community support workers; ensuring a high quality of health and safety management in line with Rehab Care policy; ensuring effective support planning and person centred planning in line with New Directions policy; supporting the manager with talent acquisition and service user referrals; engaging with all key internal and external stakeholders. A Third level graduate, Health and Social care equivalent
Participate in RehabCare training programmes in line with New Directions regulations
Experience of working with individuals with intellectual disability and mental health support needs
Experience in the provision of community and service based programmes and activities to promote independence and meet the needs, choices and abilities of clients.
Full Clean Driving Licence preferable

Your Responsibilities

To manage the day to day delivery of a person centred service for all clients who avail of Meath Day services.
To liaise with families, multidisciplinary teams and other personnel to ensure the best quality of service with the clients as directed by community services manager
To document information and complete paperwork as deemed necessary for delivery of service to the clients who avail of the Service for accountability purpose
