Significant experience in acute older persons care, discharge planning, acute rehab, frailty/FIT teams
Must have recent acute care/urgent- emergency care experience
Evidence of experience in older persons, falls & frailty, dementia, CGA a benefit.

Essential Requirements:

Recognised degree in Physiotherapy
3+ years of post graduate experience as a Physiotherapist
Excellent communication and team working skills
Above experience requirements apply
