- Collaborate with the young person, social workers, carers, and others to assess needs before age 18
- Develop a personalized Aftercare Plan six months prior to the young person’s 18th birthday
- Become the young adult’s main support person once they turn 18, replacing the social worker
- Provide guidance on housing, education, employment, budgeting, health, and relationships
- Represent the young person in accessing services like mental health, social welfare, and housing
- Bachelor’s degree in social care practice (Applied social studies in professional social care – Level 7 or 8 degree)
- CORU Registration
- Must be a resident in Ireland and have the legal right to work in Ireland without restriction.
- Willing to undergo Garda Vetting.
- International Police Clearance required (if lived outside of Ireland for more than 6 months after the age of 16).
