- Assistance with all available referral methods
- Being a direct point of contact for specific HCP queries on patient status
- Supporting the hospitals with feedback on the service from their patients
- Ensuring all activities are in line with the IPHA Code and any Data Protection regulations
- Be an ambassador for both Point of Care and our client and always operate and communicate in a professional and personable manner.
- Schedule appointments with Immunology HCPs who wish to enroll patients on the patient support programme(s).
- Provide referral and patient support materials to units, as requested.
- Demonstrate the patient support programme services available to the units.
- Assist the specialist units with enquiries on the patient support programmes.
- Demonstrate the online referral portal and enable HCP access for patient referrals.
- Assist with any online portal queries, e.g. logging in, resetting password.
- Follow up with Point of Care’s IT Department to resolve queries, if required.
- Follow up with HCPs on patient specific queries and patient case reviews.
- Identify and deliver any new or proposed service enhancements as agreed from time to time with the client.
- Be responsible for the training schedule of all units (new and ongoing) and work with the client on any training enhancements.
- Develop strong working relationships with key specialist units.
- Participate and attend relevant conferences and meetings, as requested.
- Deliver non-promotional support to patients on the client’s service offering(s) in a clinical and/or community setting.
- Share feedback and discuss potential service enhancements with both the client and the Point of Care management team.
- Perform duties in line with all regulatory, nursing and industry standards e.g. IPHA Code of Practice.
- Perform any other additional tasks, as required.
- Qualified RGN and registered with the Nursing and Midwifery Board of Ireland.
- Minimum of 3 years post registration experience in an acute hospital setting.
- Experience of working with Patient Support Programmes and in the area of Immunology is desirable (Gastroenterology, Rheumatology, Dermatology).
- Experience in patient case management.
- Customer service oriented.
- Exceptional verbal and written communication skills with strong interpersonal skills.
- Ability to educate, communicate and present while building a rapport with a range of key stakeholders.
- Excellent organisational skills and an effective time manager.
- A positive ‘can-do’ attitude with a high level of self-motivation, demonstrating the ability to work under own initiative.
- Ability to work independently and as part of a team.
- Work to the highest clinical standards by ensuring patient safety in adhering to all relevant Standard Operating Procedures.
- Excellent computer skills and knowledge of databases.
- Full clean driver’s license.
