The purpose of the post is to manage the provision of a social work service within a multidisciplinary context and in so doing, ensuring a person centred service to individuals and families.
  • Assist the progression of St Joseph’s MHIDS Transition Plan - Time to Move On from Congregated Settings – A Strategy for Community Inclusion in conjunction with Disability Services.
  • Support the implementation of the National MHID Model of Care
  • Provide a comprehensive assessment service to clients and formulate Safeguarding Plans and Care Plans in conjunction with the client, the family and the Multi-disciplinary Team
  • Assist the implementation of current legislation relevant policies and procedures guidelines and protocols i.e. Safeguarding Vulnerable Adults Policy and Children’s First.
  • Have significant knowledge and understanding of the complexities, dynamics and theories of Multi-disciplinary team work within a Community setting
  • Assessment of abuse of a physical, emotional, sexual or financial nature, together with their members of MHID and other relevant services.
  • Supported decision-making models in care planning; replacing best interest assessments with will and preferences through advanced healthcare directives.
  • Family-focused practice and mental health services – involving families and carers in the delivery of care for the service user
  • Clinical case management – responsibility for coordinating care for a caseload of service users, organising home visits, supporting families and carers on management strategies, psychoeducation and liaising with respite or day services and inpatient Mental health staff, as required. To provide liaison in respect of interagency work.
  • Role of advocate/inter-agency work/mobilisation of resources -encouraging individuals to advocate for themselves and or acting as an advocate where needed at times, in conjunction with national advocacy services and other statutory advocacy services.
  • To provide line management, clinical supervision and consultation to staff, as required.
  • The post holder will be responsible for the effective delivery of a quality social work service responsive to the needs to the service users and their families and that functions effectively as an integrated element of the wider Health Services area, inclusive of multidisciplinary PPGs.
  • To input and implement service developments with particular focus on social work services.
  • To input into CHO DNCC projects directly related to the development, reconfiguration, and improvement of Mental Health Services.
  • To input directly into the development, implementation and review of CAPAs and other duties as required in order to respond in a timely manner to the requirements of the Mental Health Commission.
  • Work in collaboration and partnership with external providers
