- Manage a diverse caseload, addressing both individual and family needs.
- Advocate for the rights and entitlements of individuals, ensuring they are informed and supported.
- Facilitate advocacy sessions, guiding individuals through self-advocacy and group advocacy initiatives.
- Identify and address unmet needs, working closely with the team to develop appropriate solutions.
- Participate in care planning and multidisciplinary meetings, collaborating with staff and clinicians to enhance support services.
- Provide safeguarding support, assisting designated officers in protecting vulnerable individuals.
- Contribute to funding applications and reports, ensuring accurate documentation and compliance with best practices.
- Maintain professional standards in line with current legislation and organizational policies.
- Recognized Social Work qualification(NQSW/CQSW) or equivalent.
- CORU registration(or in the process of registering).
- Minimum one year of professional experience in social work.
- Experience working within a multidisciplinary team.
- Strong IT skills for documentation and reporting.
- Full, clean driving license and access to a car.
