As a Support Coordinator, you will work in close partnership with each person, their families, and chosen supporters to co-create and maintain individualised service arrangements that reflect their will and preferences.Terms:This is a Permanent, Full-Time position based in Celbridge HUB. 35 hours per week Monday-Friday.Essential Criteria· A recognised third-level qualification in Social Care or a related field· Minimum of three years’ experience supporting people with intellectual disabilities in a community setting· CORU registration· Knowledge of New Directions, Social Role Valorisation, Supported/Self-Directed Living, and the Assisted Decision-Making (Capacity) Act 2015· Demonstrated commitment to person-centred, rights-based support and inclusive practice· Understanding of the National Framework for Person-Centred Planning and its application in service delivery· Experience facilitating self-directed living and meaningful community participation· Proven leadership, organisational and communication skills· Ability to work in partnership with individuals, families, and external stakeholders· Full clean driving license and access to a vehicle
