Our client is seeking an experienced Nurse with a passion for delivering high-quality Stoma care services within the community. The ideal candidate will possess relevant clinical skills, along with the drive and commitment to excel in this role. As a Stoma Care Nurse, you will work as an independent practitioner, demonstrating self-motivation and a strategic mindset. You will be instrumental in developing and delivering their Stoma care services, while also enhancing your commercial awareness and collaborative skills.Responsibilities:
- Provide professional and ethical care to patients requiring Stoma care services.
- Support patients’ transition from hospital to home by conducting home visits and community-based clinics.
- Conduct virtual consultations via telephone and video platforms.
- Efficiently plan, manage, and organise caseload and time to ensure optimal patient care.
- Communicate effectively with patients, caregivers, and referring healthcare professionals, providing feedback as necessary.
- Maintain accurate clinical records in compliance with company, legal, and professional standards.
- Establish working practices to facilitate collaboration between different stakeholders, including acute and community healthcare teams.
- Foster and maintain strong working relationships with healthcare professionals across the region.
- Deliver educational support to fellow healthcare professionals.
- Participate in service audit and evaluation activities to ensure continuous improvement.
- Registered General Nurse.
- Bachelor of Science in Nursing degree or working towards completion.
- Stoma care experience is desirable but not essential.
- Strong interpersonal and relationship-building skills.
- Proficiency in IT.
