- Conduct comprehensive customer risk assessments and develop person‑centred care plans.
- Provide support, supervision, and guidance to Care Staff.
- Monitor and uphold care quality standards across community services.
- Support and manage the performance and practice of Care Assistants.
- Offer professional expertise to enhance existing care arrangements.
- Work closely with the Clinical Nurse Manager and Director of Care.
- Deliver excellent, evidence‑based care aligned with individual care plans.
- Mentor and support community-based Care Staff, including conducting supervision visits in clients’ homes.
- Provide direct client support when required. (This should be very minimal)
- Promote and maintain best practice standards in care and customer welfare.
- Registered Nurse (NMBI) or Social Care Worker (CORU) – essential.
- Minimum 2 years post‑registration experience; community care experience is an advantage.
- Strong interpersonal, communication, and leadership skills.
- Demonstrated kindness, respect, and compassion in care delivery.
- IT proficiency required.
- Full driving licence essential.
- Full-time, permanent role (40 hours per week, will be occasional Saturday daytime hours to support weekend only clients & staff).
- Predominantly Monday–Friday, daytime hours.
