- Conduct comprehensive psychosocial assessments and develop individualized care plans for older adults.
- Provide therapeutic interventions and support to patients and their families.
- Collaborate with multidisciplinary teams to deliver holistic care.
- Advocate for patients’ rights and access to services.
- Maintain accurate and up-to-date case records and documentation.
- Participate in team meetings, case conferences, and training sessions.
- Minimum of a level 8 degree in Social Care.
- 3 years post qualifying experience to work at Senior Social Worker level 8
- CORU registration.
- Experience in mental health services, preferably with older adults.
- Strong assessment, intervention, and case management skills.
- Full driving license and access to own car is desirable.
- Excellent communication and interpersonal skills.
- Resident in Ireland and legal right to work in Ireland without restriction.
- All candidates will be required to undergo Garda Vetting.
- International Police Clearance (if lived outside of Ireland for more than 6 months after the age of 16).
